You will need to prove lineage to be eligible to purchase a death certificate.  For example, you would need to show your birth certificate to get a copy of your mother's death certificate.


To request a copy(s) of a Death Certificate fill out a Request Form, the following information is needed:

  1. Name of the decedent.
  2. Date of death.
  3. State your relationship to the name on the death record being requested.
  4. Number of copies requested.
  5. Please include a daytime telephone number.
  6. Please make the check payable to: "Town of Raymond".
  7. Who has access?
  8. Picture I.D. requirement(s).

The fee for one certified copy is $15.00. The fee for additional certified copies of the same record is $6.00 each. Send self-addressed stamped envelope with your payment.
